Incorrect billing

I was charged $12.98 in my latest phone bill for a iQIYI Premium VIP Subscription which I did not subscribe to. Initially, I renewed my contract for a SIM Only 1-year Plan and was offered free 2 months of iQIYI Premium VIP Subscription which I subscribe to. I have already unsubscribe before the expiry date but the subscription amount was automatically charged in my phone bill without my knowledge. Please assist me regarding this matter.
